Medical History Forms are required to be completed by all students upon admission to Briar Cliff University.

For your convenience, click on this link, Medical History Form, to print the medical history form.  Complete and sign it on the reverse side of the form, then turn it into Monica Arens in the Health Office in Heelan Hall 057.  A student physical is not required by the health office.
